Performed follow up x-rays and a bandage change on patient and checked the incision. Patient was placed under general anesthesia (isoflurane via mask) for approximately 30 minutes and the heart rate and respiration were monitored. The patient did not have any significant swelling on the foot. The small irritated area on the right digit has shown improvements of healing since last bandage change. The irritated area on the medial right thigh has been healing well, so we just placed a Tegaderm bandage over it. A small 2mm scab was noted at the leading edge of the distal right wing tip. The incision site looked normal. We did physical therapy with the right leg for approximately 5 minutes. We then removed the sutures and wrapped with cast padding, re-placed the thermoplastic splint (in a bent position) and re-placed Vetrap. Patient did well under anesthesia and recovered without any complications.

2 week post operative radiographs were taken, positioned in extended right lateral and extended right caudocranial. The radiographs showed good bone alignment. The fixation plate and screws are appropriatley positioned. Healing is noted, however, continued external splint support is recommended until bridging callus formation is observed across the anterior fracture gap.

We cleaned the incision with betadine and put some triple antibiotic ointment on it. We did physical therapy with the right leg for approximately 5 minutes. We placed one Tegaderm bandage on the site, wrapped with cast padding, re-placed the thermoplastic splint (in a more bent position) and re-placed Vetrap. Patient did well under anesthesia and recovered without any complications.

Took the patient to surgery. The patient was pre-medded with buprenorphine 0.1 mg/kg and midazolam 1.1 mg/kg and kept on isoflurane throughout the surgery. The patient's right leg was plucked of feathers from the stifle to the tarsometatarsus and prepper with povidone iodine. A lateral approach was taken to the right tibiotarsus. Performed a wedge osteotomy (30 degree wedge) of the right distal diaphysis of the tibiotarsus with an oscillating bone saw. Put in 2 condylar cross pins for stabilization, 1 of which was removed prior to closure. Placed a 1.5 mm LCP condylar T-plate on the caudal tibiotarsus and inserted 6 locking screws, 2 of which were unicortical (the most proximal and the middle-most) and 4 of which were bicortical. 4 screws were placed on the proximal side of the fracture, 2 on the distal side of the fracture and none within the fracture itself. There were 2 pins (0.035 in K-wire applied in an anti-rotational fashion) on the distal side of the fracture. The unicortical screws were used to prevent bone splitting. Took intra-op radiographs and fluoroscopy with a C-arm and post-op radiographs in radiology to confirm the conformation was restored. The surgical site was closed in 3 layers-- muscle, fascia and skin-- all with 4-0 PDS in a simple continuous pattern. A thermoplastic splint was placed from the just caudal to the stifle all the way down to the plantar aspect of the foot. The patient generally did well under anesthesia. However, IV atropine was given 3 times during surgery at 0.07 mg/kg the first time and at 0.04 mg/kg the second and third time. Cefazolin was given IV at 45 mg/kg twice during the surgery. The patient was additionally given IV fentanyl post-op for pain at 0.03 mg/kg. The alignment was confirmed to be improved following surgical manipulation according to the images acquired. The patient was put on post-op TMS. We will check the incision and do a bandage change tomorrow under anesthesia. We will do follow-up radiographs in 2 weeks. The pin (and potentially plate) will need to come out with dynamic de-stabilization when radiographs show 3 out of the 4 cortices show bridging across the fracture gap.

Treatment plan for 044:

Dr. Oster spoke with Dr. Corriveau this am and she said that she believes there will be enough bone to pursue a wedge osteotomy with cross-pins. According to Dr. Oster, Dr. Corriveau stated that she would like to wait until 044 is fully grown. The plan moving forward per Dr. Oster is to do re-check rads on 03/17 and 03/24 to monitor progression of the deviation if any changes take place. Dr. Oster suspects that growth plates will close at some point next week. If surgery is pursued, it would take place during the week of 03/24 after the rads. Dr. Oster stated that a decrease in handling with 044 in the next couple of weeks will take place and we will just continue to make sure 044 gets fed and medicated.

Patient was taken to VTH and anesthetized with iso via facemask. Respiratory rate and heart rate were monitored while the patient was under anesthesia. External coaptation was removed. The patient had a smooth recovery. The views that were taken include pelvis VD and pelvis R lateral. The views were taken without external coaptation. Medial deviation was noted on rads at the tibiotarsal joint. Caudal deviation was noted at this joint as well. Angulation was better than before surgery on 02-27, but was more deviated than rads taken on 03/03. It was unclear whether briding was present or not but callus formation is in progress. The current splint was removed and the R leg was examined. The toes on the right foot were less swollen than during previous days, there was medial deviation at the tibiotarsal joint, the skin and incision looked normal. There was mild inflammation present, a fibrous callus was palpated, which is suspected to be mostly fibrous but there could be a bony component as well. ROM of the foot was done. A new splint was made. The plan moving forward is to do re-check rads on 3/10 and unlesss major abnormalities are found, external coaptation can be discontinued.

Removed bandage and splint from R leg. Toes remain mildly swollen, no change over past 3 days. Removed the tegaderm+pad from the incision - no blood or discharge present. There is a large, firm callus present at the fracture site. Bruising has nearly disappeared, all skin appears pink and viable. Did PROM of the R hock joint and massaged toes. Reapplied bandage and splint. Will need to make new, larger splint on Friday. With current rate of healing, suspect can discontinue coaptation on 3/10.

Bandage change 044

While under anesthesia via iso at VTH for rads, the splint bandage on the right leg was removed. Heart rate and respiratory rate were monitored under anesthesia. There was no devitalized tissue present on the leg and the incision site looks good. There is inflammation present, but this is expected. A new splint was made and placed due to 044 outgrowing the splint that was on previously. There was some lateral deviation present at the tibiotarsal joint, but the cranio-caudal segment looked good.

Surgery report
Procedure - Fracture repair - tibiotarsus
Performed by - Dr. Sokoloff
Surgery time - 0 minutes

Patient 044 was anesthetized and placed in dorsal recumbency
in the OR. The right leg was plucked, scrubbed, and isolated
via drapes and tape on the surgical table. A scalpel blade was
used to make a small incision on the plantarolateral aspect of
the tibiotarsus. The skin and subcutaneous tissue layers were
exposed via the scalpel blade. A bony callus was encountered
around the fracture site on the plantar aspect of the distal
tibiotarsus under the subcutaneous layer. The callus was isolated
and filed down until the bone was smooth to allow the tibiotarsus
and tarsometatarsus to be in a more stable alignment. A C-arm
radiography machine was used throughout the procedure to visualize
how much bony callus was left to be removed. Manipulation of
the joint was done at different stages during the procedure to
determine mobility and feel how much bony callus was left. Once
it was determined that the joint was mobile enough and the fracture
could be stabilized, the subcutaneous tissue layer was closed
using 5-0 PDS in a simple interrupted pattern. The skin was also
closed using 5-0 PDS in a simple continuous pattern. The sutures
were covered with a teflapad as the primary contact layer and
then wrapped using cast padding as the secondary layer. A thermoplastic
splint was then placed with the right leg in weight bearing position,
determined by comparing it to the non-fractured left leg. The
external fixation was completed by a layer of vet wrap. The patient
was then taken to recovery and woke up from anesthesia without
any complications.

Dr. Sokoloff did a physical examination on 044 at 11:40 am on 2/25. The patient was bearing more weight on the left leg than the right. Upon examining the radiographs, she suspects a folding fracture in the R tibiotarsus, distal diaphyseal approaching the growth plate. She also noted an abnormal curve proximally to the stifle in the rads. Dr. Sokoloff discussed reaching out to the Minnesota Raptor Center for consultation. 044 is scheduled for surgery to re-fracture and externally stabilize on Thursday 2/27 at 1 pm. An anesthesia report was turned into the anesthesia department at the vet school this afternoon.
